Are you editing the file in notepad? I was, and it cause this error even if the file was BLANK. I figured that it might have to do with the way that notepad saves the file so I opened the custom_functions.php into an editor (Netbeans).. saved it.. and everything worked 100% I had a major headache with this problem as well..
